Texas Valero petroleum refinery explosion killed 1, injured 2

Witherite, reports a deadly refinery accident at the Valero petroleum refinery facility in Texas City, left one worker dead and left two others injured. The fatal workplace accident occurred on Friday, December 4, 2009 around 9:00 p.m., as reported by the Houston Chronicle.
A crew of workers was reportedly attempting to restart a boiler at the Valero refinery facility, which went offline early that day, when the deadly explosion occurred. The violent explosion fatally injured instrumental and electrical operator, Tommy Mannis, 40, of Alvin, and injured an unidentified Valero employee, and a contract worker. The unidentified Valero employee was rushed by emergency medical service (EMS) teams to Galveston area hospital, while the other worker was taken to a Texas City Hospital for treatment. Their injuries and current state of condition were not reported. The boiler the men were working on apparently makes steam to power other units at the plant. Occupational Health and Safety Administration (OSHA) http://www.osha.gov/ investigators and Valero officials are conducting a full investigation into the deadly refinery blast. It is currently unknown what exactly caused the explosion, or if anyone involved has obtained legal attorneys.
